Consent Forms for Oral Medication at School
Under Procedure VI-68, if your child requires oral medication during the school day, the following consent forms must be completed before school staff can administer any medication.
- Administration of Prescription Oral Medication Form – for any oral medication prescribed by a doctor.
- Administration of Non-Prescription Oral Medication Form – for over-the-counter oral medications such as pain relievers or allergy medication.
Instructions for Parents/Guardians:
- Download and print the applicable form(s) below.
- Complete all sections and ensure the form is signed.
- Return the completed form to your school’s main office before your child requires the medication at school.
