The online forms to request a change or confirm your child鈥檚 learning environment in elementary school (i.e. virtual or in-person) are now open.
Students Currently Learning 鈥 In-person Instruction Only
If you wish to change your child鈥檚 learning environment by switching from in-person learning to virtual (remote) instruction for the current 2021-2022 school year, you are asked to complete an online request form.
This online request form applies to elementary students currently learning in-person ONLY.
Important notes before you complete the online form:
- You are asked to submit a separate form for each child.
- You will need your child鈥檚 Ontario Education Number (OEN) to submit this request. The OEN can be found on a recent report card.
- This form does not apply to homeschooling. If your child is currently being homeschooled and you would like to request a change to either in-class instruction or remote learning, please contact your Family of Schools Superintendent.
- The decision you make on this form will remain in effect until the end of the school year.
NOTE: Please do not complete this form unless you want to change your child鈥檚 current learning environment from in-class instruction to virtual (remote) learning in a virtual classroom at .
The online form will remain open until 11:59 p.m. on Friday, April 1, 2022.

Once the online form has closed, our staff will begin to work through the rescheduling process. The changes will come into effect on Tuesday, April 19th, 2022.
A Note to Parents with Children in Secondary School
If you also have one or more children registered in an HCDSB secondary school, please note that students may move to virtual learning at any point throughout the school year. Students may move from remote to in-person learning at the final designated re-entry date for this school year: April 22, 2022. Please contact your secondary school for more details.
Students Currently Enrolled at St. Clare of Assisi Virtual Catholic Elementary School
In January, we asked parents of students enrolled at to choose whether they wanted to keep their child(ren) enrolled in virtual learning until Easter OR until the end of the school year.
On March 9, 2022, the Ministry of Education lifted a number of measures that were previously in our schools, including mandatory masking, cohorting and physical distancing. These changes came into effect on Monday, March 17th.
Understanding that the recent changes may impact parents’ decisions, we are now asking parents to confirm their choice of learning environment for child(ren) for the remainder of the 2021-2022 school year.
